Begin the insurance claim process before your tattoo appointment. Starting early can reduce waiting time for reimbursement and ensure you have all necessary documentation ready. Starting at the time of your consultation is best to have all information necessary.
Contact your insurance provider first, & ask about their requirements for coverage and reimbursement for medical 3D areola tattooing.

Not all insurance companies will approve claims for this procedure, so confirm eligibility in advance.

We are not an in-network provider for any insurance company. However, some insurers may allow a one-time in-network application, approving us for their qualification.

We do qualify for PPO insurance plans.
You will need to provide several documents to support your claim:

Prescription from your doctor with the correct codes

Claim reimbursement form from your insurance company

Receipt of payment for the tattooing procedure (including any deposits)

Letter of medical tattooing necessity (your tattoo artist can often provide this)
Submit your claim with all required documentation.
